Community Dental Care data breach settlement

A December 2024 cyberattack on Community Dental Care, a Minnesota non-profit dental provider, reached files containing names, dates of birth, Social Security numbers, health insurance details and medical information. The settlement class is everyone whose personal information was accessed without authorisation, including everyone sent a breach notice. If you were notified you may be eligible for a flat $50 with no paperwork, or instead for documented unreimbursed losses of up to $5,000, and in either case two years of medical and credit monitoring. The claim deadline is October 19, 2026, and filing on the official settlement site is free.

When the money moves

What the official materials say about the payment schedule for this settlement, and what the administrator has published since. Every figure below is linked to the page it was read from.

Expected

The court is due to consider final approval on November 23, 2026, and payments follow approval and any appeals.

Read off the official notice, in its own words. These dates slip — an appeal can move a distribution by a year or more — so treat it as the administrator's plan rather than a schedule.

In re Community Dental Care · 62-CV-25-2490 · District Court for Ramsey County, Minnesota

You choose one of two cash options: a flat $50 that needs no documentation, or reimbursement of unreimbursed out-of-pocket losses traceable to the breach up to $5,000, which does need bank statements or receipts. Either way the class may also take two years of CyEx Medical Shield Complete with $1,000,000 of medical identity theft insurance. The settlement site does not publish a fund size, so the amounts above are ceilings and estimates rather than guaranteed payments.

Eligibility questions

These questions help you decide whether to read the official notice. They are not a legal determination.

Did you receive a notice that your information was involved in the Community Dental Care data breach?
Were you or your child a patient of a Community Dental Care clinic?
Did you pay anything out of pocket because of the breach, with receipts or statements to show it?

Documented losses can be claimed instead of the flat $50.
