Valley Oaks Health data incident settlement

Valley Oaks Health, an Indiana behavioural health provider, has settled a lawsuit over a June 2023 cyberattack that may have exposed names, dates of birth, Social Security numbers, diagnoses and clinical information. The class is everyone on the settlement class list whose private information was potentially compromised, so if you were notified you may be eligible for a flat $40 payment with no proof, or instead up to $500 in ordinary documented losses, up to $5,000 in extraordinary losses and up to $80 for time spent, plus two years of medical identity monitoring. Claims close November 9, 2026, and the court decides whether to approve the settlement on December 10, 2026. Filing on the official settlement site is free, and only the administrator and the court decide who is paid.

When the money moves

What the official materials say about the payment schedule for this settlement, and what the administrator has published since. Every figure below is linked to the page it was read from.

Expected

The notice says benefits are distributed after the court approves the settlement at the December 10, 2026 hearing and any appeals are resolved, without naming a payment date.

Read off the official notice, in its own words. These dates slip — an appeal can move a distribution by a year or more — so treat it as the administrator's plan rather than a schedule.

The no-proof option is a flat $40. In its place a class member may claim up to $500 in documented ordinary out-of-pocket losses, up to $5,000 in documented extraordinary losses from theft or fraud, and up to $80 for lost time at $20 an hour for up to four hours. Receipts are required for the loss options and the notice says personal notes alone are not enough. Two years of CyEx Medical Shield Pro monitoring with $1 million of identity-theft insurance is also offered. The online form opens only with a Login ID and PIN from the mailed notice; a paper form can be mailed instead.
