Institute for Human Resources and Services data breach settlement (Tereska)

The Institute for Human Resources and Services, a Pennsylvania provider, settled a case over an August 2025 breach that the notice says exposed names, dates of birth, Social Security or tax ID numbers, driver's license numbers and medical information. People in the United States whose information was accessed may be eligible to pick one cash option: a flat $45.00 payment needing no documents, or up to $2,000.00 for out-of-pocket losses traceable to the breach and incurred between August 13, 2025 and December 2, 2026, supported by statements or receipts. Two years of credit monitoring with $1 million of fraud insurance is offered separately, through an enrollment code the administrator sends. Claims are due December 2, 2026 and the online form asks for the LoginID and PIN from the mailed notice, though a paper form can be posted instead; filing is free either way, and the court hears final approval on December 17, 2026.

Expected

The FAQ says payments are distributed if the court grants final approval at the December 17, 2026 hearing and after any appeals are resolved. No distribution date is published yet.

Read off the official notice, in its own words. These dates slip — an appeal can move a distribution by a year or more — so treat it as the administrator's plan rather than a schedule.
