HCF Management data incident settlement
This settlement covers people whose personal information was caught up in the data incident at Ohio nursing-home operator HCF Management in September 2024. A claim is either a flat $35 in cash or documented losses — up to $400 in out-of-pocket expenses, up to $80 for lost time and up to $4,000 for extraordinary losses — and everyone in the class can also claim three years of credit monitoring. The window is short: claims close September 29, 2026, and the online form asks for the Login ID and PIN printed on the mailed notice, though the administrator will reissue those by email and a paper form can be mailed instead. If you believe you meet the criteria you may be eligible, and filing on the official site is free.

Expected
The settlement FAQ says payments go out as soon as possible after the court's final approval order becomes final, and that an appeal can delay that; the approval hearing is set for November 30, 2026.
Read off the official notice, in its own words. These dates slip — an appeal can move a distribution by a year or more — so treat it as the administrator's plan rather than a schedule.
